Washington State Coalition for Language Access Language Access Barriers Documentation Form
WASCLA is a state-wide coalition of advocates, agency staff, and interpreters working together to improve language access to Limited English Proficient individuals in our state.
As part of achieving this goal, WASCLA is gathering information on language access barriers, including examples or incidents, as well as descriptions of larger systemic problems, such as lack of training or resources.
The form should be used to document problems encountered by Limited English Proficient individuals, service providers or agencies that are experiencing problems with services. It can be copied and distributed as necessary.
Please document only one incident per form.
